Yep, that's simply because they don't have the stuff needed to emulate puke bag zone in the original genesis game, so they went for the next best thing.
This also appears in the Sega Master System version, too!
@RainbowQuestionMark: It's actually based off of the Special Stage from the Game Gear version of the game.
That looks kinda different than the original Sonic for the Genesis.
